DRAFT***Student Health Insurance***DRAFT

In order to ensure that students have access to services and help they need, and to minimize disruptions to their academic careers, YU has a student health insurance requirement. Below is some information about the requirement and some frequently asked questions.

Requirements

Generally, all students (both undergraduate and graduate) who are registered for 6 or more credits are required to either enroll in the student health insurance plan provided by the university, or provide proof of comparable coverage and submit a waiver request to avoid being under the university plan.

Students who are registered for less than 6 credits, or who are part of a program which has been designated as entirely online, are not eligible for/exempt from having to waive or enroll. NOTE: The program has to be designated as entirely online – if the program is not, even though you may have semesters where all courses are online, you would still have to either enroll in or waive out of the student health insurance plan.

All communication from the university and from Gallagher will be sent to the student’s university email account.

The health plan is an Aetna Health PPO, providing full coverage for students who enroll, as well as eligible dependents who can be included.

The insurance plan is annual, running from August 15th of one year to August 14th of the next. For students who enroll in the Spring semester, the coverage period is from January 10th to August 14th For students who enroll in the Fall and then graduate or leave, it runs from August 15th to January 9th.

All students who are required to either enroll or waive must take action by either submitting an enrollment request or a waiver request.

The enrollment/waiver period runs until October, usually starting in the summer. It is important to note that if a student does not take any action, they will be automatically enrolled in the student health insurance plan at the end of the waiver period, and they will be charged. Additionally, if they do want the health insurance and want it to be active right away, they will need to submit an enrollment request – otherwise the insurance will not start for them until after the waiver period, and they will be charged and responsible for the full amount still.

YU has contracted with Gallagher Student Health and Special Risk to help administer the student health insurance plan and interface with Aetna Health. All enrollment and waiver requests have to be submitted through and reviewed by Gallagher.

Students will go to Benefits Dashboard - Gallagher Student Health and Special Risk and log in using their Inside Track username and password. Once logged in, they will see the options to submit the enrollment or the waiver request. It is recommended to use Google Chrome when accessing the Gallagher website.

Students will need to fill out the information for themselves (and any dependents they want to enroll) and submit the enrollment request.

Students can enroll their eligible dependents (such as spouses/partners or children) for the duration of their coverage each year. NOTE: All dependent coverage payments are made through Gallagher directly, while the student/primary coverage is included as part of their tuition and fees and charged by the university.

Depending on whether all the information is needed, the request can be processed between 24-48 business hours and up to a business week. For students who enroll in the student health insurance plan, the ID card may take 5-7 business days to be available once the request is processed.

For the 2022-2023 school year, the charges are as follows: Student** $4,750.00 for the annual coverage $2,834.00 for those who enroll starting in the spring Spouse/Domestic Partner* $4,725.00 for the annual coverage $2,809.00 for those who enroll starting in the spring Each Child* $4,725.00 for the annual coverage $2,809.00 for those who enroll starting in the spring 2 or More Children* $9,450.00 for the annual coverage $5,618.00 for those who enroll starting in the spring Spouse and 2 or more children $14,175.00 for the annual coverage $8,427.00 for those who enroll starting in the spring (*) Yeshiva University splits the annual cost by charging 50% of the annual premium in the fall and 50% in the spring. (**) The student rate includes the $25.00 annual administrative fee charged by Yeshiva University NOTE: The dependent costs are in addition to the primary/student coverage. Dependent costs must be paid directly to Gallagher when submitting the enrollment request, while primary/student costs are included with tuition and fees and must be paid through YU.

It pays for a wide range of medical services, including hospital room and board, inpatient and outpatient surgical procedures, labs and x-rays, chemotherapy and radiation, inpatient and outpatient mental health services, physician office visits, consultant visits, ambulance, emergency care and prescription drugs. It also covers intercollegiate sports injuries the same as any other Injury. The plan also covers preventive care services at no cost when you use In-Network providers. This includes routine physicals and examinations, screenings, GYN examinations and most immunizations. It pays for 80% of your medical fees when you use In-Network providers. This is the advantage to using in-network providers, as using Out-of-Network providers the coinsurance is less which means the potential out-of-pocket costs are more. Enrolled students will also be responsible for paying any deductibles and applicable office visit and prescription drug copayments. There is a $550 per Insured, per policy year deductible for In-Network services and a $2,000 per Insured, per policy year deductible for Out-of-Network services. There is a $45 copayment for office visits Prescription drugs are covered for a 30-day supply after a: • $25 copay for a Tier-1 drug • $70 copay for a Tier-2 drug, • $100 copay for a Tier-3 drug. (Note: in some cases, prescription drugs may have a supply or quantity limit or require your doctor to get a Prior Authorization before you can pick-up your prescription. Generally speaking, the coverage being used to request to waive out of the student health insurance plan offered by the university has to be comparable and active as of the beginning of the coverage period for the student health plan. When filling out the waiver request, students will need to have their insurance information in order for a request to be complete and submitted for review by Gallagher.

Gallagher will review the request and email once it has been received, if there are any updates or additional information, and when the request is approved/denied. If the request is approved, Gallagher will provide an update to both the university and the student, and the university will update the student account to remove the charge, which can take 24-48 hours and up to a business week for everything to be fully processed. If you have successfully waived out of being enrolled in the student health insurance plan, you are confirming that you think your coverage will provide you with enough care and services to meet your needs, and that Yeshiva University is not responsible or liable in the case that your coverage does not fully provide a comparable level of service and care. If you have successfully waived out but decide you want to enroll later, you will need to meet a Qualifying Life Event and provide proof in order to do so. Qualifying Life Events include: • Reaching the age limit of another health insurance plan • Loss of health insurance through marriage or divorce • Involuntary loss of coverage from another health insurance plan
