    The YESS system requires that each departmental administrator complete this form. The form is used to set up accounts for people in your departments that will need to enter, view or approve PRF's, PM's and PSF's.

    Application Instructions 

    Fill out the form below starting with the requestor information. One complete form must be completed for each department under your authority.

    A copy of the form will be emailed to you at the email address that you used in the form.
