Request Forms

The following forms may now be filled out online and then printed and/or faxed to ITS Help Desk. The Help Desk fax number is 212-960-5454.

Mainframe, Banner, and Departmental File Servers

•  Authorization for Mainframe Systems Access or Banner Systems Access This form is required for each individual needing access to any application on Banner or the mainframe and includes a connection to the university network. Please check the application(s) and the type(s) of access required. In addition, make sure your IP address is entered on the form. The form may be filled out on the computer or printed and filled out off-line. The form must be printed ultimately for signature by the Department Head and by the Director responsible for control of the data. It will then be processed by the ITS department. 
• Departmental File Services System Information and Enrollment FormThe ITS Department has established a centralized file services system for use by administrative departments. This facility, located on the Wilf Campus in the Belfer Hall Data Center, provides a secure, professionally administered environment for housing and sharing data files. This system can be accessed from anywhere within the university's administrative networking infrastructure. Departments not currently using a file server may apply for this service at any time.
