Procedure for Special Services

Requests for special services, other than for emergencies, must be submitted on a Request for Services Form to the Department of Facilities Management. (Available on this site.) This form should be as clear and precise as possible, listing all pertinent information, and be signed by an authorized individual. The requesting department will be notified regarding the action to be taken upon their request, with an estimated completion date.

Requests for special services should be submitted at least seven (7) working days before they are needed. Major work will require a longer lead time as will work requiring the ordering of special equipment. All work will be undertaken as funds and staff permit.

Charges

Charges for normal routine repairs and services are included in the annual institutional support, space costs and related "overhead" costs allocated each year for the budget purposes on the basis of a formula which, among other factors, includes the square footage used by the department. There is an additional charge for special requests and services. Such costs are determined by the price of supplies and materials used to complete the request and hourly charges for the labor involved, including supervision. Premium charges will apply for requests that are not submitted seven days in advance and where overtime is  involved.

Special requests that are estimated to have costs in excess of $500.00 must be approved by the budget office before they are submitted to Plant Operations. Such approval must be noted on the Request for Service Form.
